POSITION TITLE: Pharmacist

SITE: Toronto Western Hospital (TWH)

REPORTS TO: Outpatient Pharmacy Manager

START DATE: As soon as possible

STATUS: Casual

HOURS: Part-time (up to 0.8 FTE or 4 days a week)

The PHARMACIST provides comprehensive pharmacy services to specialty ambulatory patients at TWH including: Musculoskeletal, Rheumatology and Ophthalmology clinics and cross covers the outpatient dispensary.

As a member of the Outpatient Pharmacy team, the principal responsibilities of the Pharmacist include:

Proactive review of prescription and specialty medication orders for appropriateness; identify, resolve and prevent drug related problems with the patient and health care team; monitor drug therapy for effectiveness, toxicity and adherence to evidence based medicine/rational prescribing guidelines; conduct medication reconciliation; follow up on medication incidents and promote safe medication practices in the clinics; respond to drug information questions; provide patient education; provide direct and/or functional supervision of technician staff.

In addition, the pharmacist will support ambulatory clinic physicians and team members with pharmacological consultation, education and information that contribute to the management of chronic conditions, promote wellness of patients and the knowledge of providers. The pharmacist will facilitate program development, clinical teaching and quality improvement activities within the department, while exemplifying UHN’s values of caring, excellence, teamwork, innovation, integrity, leadership, and respect.

QUALIFICATIONS:

Education:

  • Completion of a Bachelor's of Science Degree in Pharmacy or Doctor of Pharmacy Degree required.

  • Certificate of competency from the Pharmacy Examination Board of Canada required.

  • Active registration with the Ontario College of Pharmacists (Part A) required.

  • Certificate of competency from an accredited smoking cessation program and flu vaccination injection program required.

Experience:

  • Ambulatory Outpatient Pharmacy practice experience preferred.

  • Pharmacy residency is an asset.

  • Familiarity with common drug therapy used in the MSK-Rheumatology population, as well familiarity with dispensing processes for specialty medications.

  • Experience working as part of a multi-disciplinary team, including developing the clinical role of a pharmacist in a new clinical setting.

  • Strong interpersonal, communication and customer service skills

  • Competency in providing medication reviews and MedsCheck documentation process

  • Familiarity with manual billings and specialty medication related billings are an asset

  • Experience/working knowledge of the HealthWatch System.

  • Working knowledge of adherence services including but not limited to compliance packaging, refill synchronization, and refill management

  • Experience handling pharmacy inventory

  • Membership in the Canadian Society of Hospital Pharmacists and/or other professional societies an asset.
