Hours: 30 hours per week

Status: Permanent, Part-Time

POSITION:

The Pharmacist provides comprehensive specialty pharmacy services to oncology patients and walk in clients at PM. As an integral member of the Outpatient Pharmacy Team, the STAFF PHARMACIST is responsible for ensuring prescriptions are filled accurately and timely with the patient's best interest in mind. The pharmacist must surpass the Ontario College of Pharmacists' standards of practice.

The principal responsibilities include: reviewing and processing prescription orders for appropriateness; identify, resolve and prevent drug therapy problems; monitor drug therapy for effectiveness, toxicity and adherence to evidence based medicine/guidelines. In addition to conduct best possible medication history and medication reconciliation, pharmacist also participates in both on-site and off-site patients outreach programs. Pharmacist will be in constant contact with physicians regarding patient queries. Promoting safe medication practices and specialty medication usage are activities intrinsic to this pharmacist. The pharmacist is expected to provide specialty medication consultation services.

QUALIFICATIONS:

Education:

  • Completion of university degree (Bachelors of Science Degree in Pharmacy), or equivalent required.

  • Certificate of competency from the Pharmacy Examination Board of Canada required.

  • Active registration with the Ontario College of Pharmacists (Part A) required.

Experience:

  • Hospital pharmacy practice experience in oncology preferred.

  • Hospital pharmacy residency an asset.

  • Membership in the Canadian Society of Hospital Pharmacists an asset.

  • Kroll and Healthwatch experience preferred

  • Excellent organizational and time management skills.
