Position: Social Worker
Site: Toronto Rehabilitation Institute, Lyndhurst Centre
Department: Spinal Cord Rehab Program, Unit 1A
Reports to: Manager, Program Services
Hours: 37.5 hours per week
Status: Temporary full-time maternity leave coverage approx. 6 months
In the provision of patient and non-patient care activities within the Spinal Cord Rehab Program , the primary responsibilities of the SOCIAL WORKER include: providing innovative application of social work processes in psychosocial assessment, education, advocacy, supportive and adjustment counselling with patients and families within the rehabilitation continuum, philosophy and process; conducting comprehensive psychosocial assessments independently and as part of an interprofessional team; coordinating care plans and future care planning, including appropriate referral to community agencies and support services; collaborating with the interprofessional team and community-based service providers; providing leadership related to team communication with patients and families; participating in professional, program and hospital-wide program development and meetings; participating in teaching initiatives, including student education and the patient/family SCI education program; participating in quality management and research initiatives; performing activities required for the effective and operation of Social Work practice i.e. compliance with and commitment to patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patients; performing other duties consistent with the job classification, as required.
Master's Degree in Social Work (M.S.W.)
Membership with Ontario College of Certified Social Workers and Social Service Workers (OCCSWSSW)
Membership with Ontario Association of Social Workers (OASW) preferred
Two (2) years experience working with a multi-disciplinary team in a health care setting preferred.
Experience working with adults who have sustained a spinal cord injury or illness with complex medical and psychosocial needs
Knowledge regarding community resources (i.e. housing and funding) available for people with disabilities
Excellent interpersonal skills
